Optic White Professional In Office Teeth Whitening Kit Promotion
Participation in the Optic White In Office Promotion is restricted to dentists in the United States that have been invited to participate by their Territory Manager and have purchased 4 Optic White In Office Kits. Participating dentists who use the Optic White Professional In Office Teeth Whitening Kit on a patient will be compensated $150 per post-procedure survey (up to 4 surveys). Participating dentists are required to use the Optic White In Office Kit with a patient according to the requirements of participation as follows: (1) Perform the whitening treatment within 6 weeks of accepting the invitation to participate (the start of the 6 weeks begins the Monday after the invitation was received and accepted) (2) Within the 6 week period, complete and submit a post-procedure survey for each patient (up to 4 patients) that received the Optic White In Office whitening treatment; and (3) Within the 6 week period, submit a certification of completion via a photo of the Optic White Professional In Office Teeth Whitening Kit’s lot code located on the back of the box and Colgate Account number. After completion of the above steps, Colgate will validate the submission within 48 hours. If valid, you will be sent the compensation immediately. If not valid, you will receive a follow-up email with next steps. Participating dentists with valid submissions will be compensated $150 per post-procedure survey (up to 4) within 48 hours of completing steps 1-3. Colgate reserves the right to modify or cancel this program at any time without prior notice. This discount offer cannot be redeemed by non-participating dentists and cannot be combined with any other special offers, discounts or promotions (including promotional codes).
Requirements of Participation*

TREAT
Perform the whitening procedure on a patient using Colgate® Optic White® Professional In-Office.

SUBMIT
Complete the post-procedure survey, upload a picture of your box's lot code and submit your Colgate account number to verify proof of use. You are allowed one email address per participant (Colgate account number), please ensure you use the same email address for all submissions. Please note, an email address can only be associated to 1 Colgate Account number.

An email will be sent to you confirming that your submission has been received. Once your submission has been received, your submission will be validated within 48 hours and you will immediately be compensated $150 per post-procedure survey (up to 4) for each valid submission.
